Forms
Registration forms will be available below each program description for a limited time prior to the start of each individual activity. Registration forms are also available at the Parks and Recreation building and the Public Schools for a short period of time prior to the start of each activity.
Return of Registration Forms
Any registration form which requires a payment:

Activities
Archery: Ages 7-Adult
Dana White will be the instructor for our Archery class; Dana is a Level III Certified Instructor with the National Archery Association and has been teaching archery to all ages for over 30 years. Classes will combine the basic skills of Archery combined with all aspects of safety and fun to make each lesson interesting. Archery helps build attention skills, focus, respect, responsibility, self-esteem and much more. Contact Dana White at 396-7979 for registration and current session information.

(NEW) Growin’ Grizzlies Sport A Day: Kindergarten Age Children
This is an exciting class to introduce the basic skills of various sports to children in Kindergarten. We will play the following sports in no particular order; soccer, T-ball, basketball, and floor hockey. This class will take place on Wednesday with a morning and afternoon session, class is limited to 10 participants per session. This will be a four week program May 2, 9, 16, and 23.

Sport-A-Day Registration Form
Coed Instructional Lacrosse: Ages 7 & 8
Instructional Lacrosse is open to boys and girls ages 7 & 8. This will be an instructional program where all rules of the game will be taught. We will focus on skill development through many different drills and fun games as well as team games against each other. The focus will be on learning, fun and sportsmanship. Participants will need to provide their own protective equipment: Helmet, shoulder pads, elbow pads, gloves, mouthguard and stick. Participants will also need to purchase a team reversible pinney to wear at practices and games.

2012 Lacrosse Registration Form
Boys Lacrosse: U11 (ages 9 & 10), U13 (ages 11 & 12), U15 (ages 13 & 14)
Boys Lacrosse is open to all boys ages 9-14 (age as of 8/31/11). All players will be assigned to a team dependent upon player registration and age. This is a competitive travel program yet we will stress skill development, sportsmanship and fun. All abilities are invited to participate. Games will be played on Sundays with practices throughout the week. All participants must provide their own protective equipment including: helmet, shoulder pads, elbow pads, gloves and mouthguard. Participants will also be required to purchase a team uniform (approx. $50) this will be theirs to keep. Program will begin mid-March. Look for registration info in early January.

2012 Softball Registration Form
Girls Minor League Softball: Ages 9-11
Minor League Softball is a competitive yet instructional program. Participants will be taught the fundamentals of the game, we will stress learning, fun and sportsmanship. Pitching will be introduced in the Minor League. All participants will be assigned to a team with a full schedule of games and practices. Equal playing time for all participants is encouraged.

Girls Major League Softball: Ages 12-16
Major League Softball is a competitive yet fun program. Participants will be taught the rules of the game we will also stress learning, fun and sportsmanship. All participants will be assigned to a team with a full schedule of games and practices. Equal playing time for all participants is encouraged.

Ballet Recital:
The 2012 Ballet Recital will take place this spring, location and date to be announced at a later date. The Ballet Recital is a culmination of the entire year where the dancers show off the many new skills they have learned throughout the year.
Co-Ed Volleyball: Ages 16 and up
Co-ed Volleyball takes place on Wednesday evenings beginning at 7pm at Mountain View Middle School. This is an ongoing program throughout the year and the 2010 season will begin on September 29th and continue into May. Anyone 16 or older and all skill levels are invited to attend. No pre-registration is needed.

Coed Volleyball Registration Form
30+ Mens Basketball:
This is a casual pick-up program that takes place on Sundays from 6-9 pm at Mountain View Middle School. Anyone interested in playing basketball is encouraged to attend, no registration is needed. A second night of basketball is also available on Thursday evenings when the Mens Basketball League is not in session.

Yoga: All Ages
Yoga for the beginner is offered at the Parks and Recreation Building on Wednesdays from 9am – 10:30am. Registration dates for Spring Yoga will be announced at a later date.
