Forms

Registration forms will be available below each program description for a limited time prior to the start of each individual activity.  Registration forms are also available at the Parks and Recreation building and the Public Schools for a short period of time prior to the start of each activity. 

Return of Registration Forms

Any registration form which requires a payment:

3 vs. 3 Hockey Tournament

Goffstown Parks and Recreation will hold a 3 vs. 3 Hockey Tournament over February School Vacation from 2/25 - 3/4,dates and times may vary depending upon the amount of teams registered.  There will be 4 divisions: Jr. High (Grades 6-8), High School (Grades 9-12, Open Division (18 and older), and a 30 and over division.  All players must wear helmets, mouthguards, gloves & skates.  Protective equipment is optional but highly recommended.  Games will consist of 2 - 10 minute halves.  Teams can have a maximum of 4 players.  Tournament will take place at the Parks and Recreation Outdoor Rink. 

Archery: Ages 7-Adult

Dana White will be the instructor for our Archery class; Dana is a Level III Certified Instructor with the National Archery Association and has been teaching archery to all ages for over 30 years.  Classes will combine the basic skills of Archery combined with all aspects of safety and fun to make each lesson interesting.  Archery helps build attention skills, focus, respect, responsibility, self-esteem and much more.  Contact Dana White at 396-7979 for registration and current session information.

Archery Registration Form 

 

Yoga:  All Ages

YOGA: Class meets  at the Parks & Recreation building  on Wednesday mornings.  A new six-week session begins March 2 with classes meeting on Wednesday mornings 8:30 - 10:00 am. Dance:  Boys and Girls ages 3 and up

Professional dance instruction is offered specializing in ballet training.  Classes in ballet, pointe, tap, modern, jazz, hiphop are offered at the pre-school level as well as junior and senior intensive programs.  Classes for pre-school age children take place on Wednesdays and junior and senior intensive programs take place on Fridays.  All participants work towards the Spring Performance.

30+ Mens Basketball:

This is a casual pick-up program that takes place on Sundays from 6-9 pm at Mountain View Middle School.  Anyone interested in playing basketball is encouraged to attend, no registration is needed.  A second night of basketball is also available on Thursday evenings when the Mens Basketball League is not in session.

Mens Basketball League:  Ages 18 and older

This is a competitive basketball program for men ages 18 and older.  Practices and games will take place on Thursday evenings at Mountain View Middle School.  All participants will be assigned to a team with a schedule of games, the schedule will start in early October and run through February.

Co-Ed Volleyball:  Ages 16 and older

Co-ed Volleyball takes place on Wednesday evenings beginning at 7pm at Mountain View Middle School.  This is an ongoing program throughout the year and the 2011 season will begin on September 21st.  Anyone 16 or older and all skill levels are invited to attend.  No pre-registration is needed.

Season: Session I-September 21-December 21/ Session II- January 4-April 11       Fee: $20 resident per session/$30 non-resident per session                 Registration Deadline: Ongoing

Coed Volleyball Registration Form

 

Activity Period: Boys and Girls in grades 1-4

Activity Period is a fun filled program where we will play traditional and non-traditional gym games in a fun filled atmosphere.  Games will include Floor Hockey, Kickball, Dodgeball, Wiffle Ball, White Knight and many more.  This program is for grades 1-4. Space is limited to 25 participants per session – please register early.

2011/12 Activity Period Registration Form 

 Coed Instructional Basketball grades 1 & 2 -  (2012 season is Full)

This will be an instructional / fundamental program with lowered basketball hoops (8.5’) for both girls and boy’s in 1st and 2nd grade. We will focus on fundamentals, fun, and the understanding of the game. We will take a maximum of 80 players with 10 players on each team. Once the season begins we will have a 30 minute practice followed by a 45 minute game each Saturday. All registration forms must be complete prior to participation in team selection. Financial aid is available, contact the Parks and Recreation Office for further information.

Day/Time: Clinic Dates - 12/3 8am-9:15am & 12/10 8am-9:15am

Starting in December participants will have a combined weekly practice and game on Saturday mornings from 8am-9:15am

Girls Jr. Basketball: Girls Grades 3, 4 & 5

This program is for girls in grades 3, 4 & 5.  It is an instructional yet competitive program all abilities are invited to participate.  All aspects of the game will be taught in a fun and instructional environment.  There will be a 1 hour practice during the week and a practice on Saturday morning before the season begins.  Games will begin in December. All girls will be assigned to a team as long as they register by the deadline. All registration forms must be complete prior to participation in team selection. Financial aid is available, contact the Parks and Recreation Office for further information.

 Pre-Season Clinic Dates - 11/19 12pm-1:15pm, 11/29 6:30pm-7:45pm & 12/3 12:30pm-1:45pm

Day/Time: Monday-Wednesday with pre-season practices on Saturdays.  Practice 1 night a week with a game 1 night a week starting in December.  Games are scheduled to start at either 6:30pm or 7:30pm.  Practices will be 1 hour during the week, location and time may vary.

Location: MVMS or Maple Ave. – All games at MVMS Gym

Boys Jr. Basketball:  Boys Grades 3, 4 & 5

Practices will be 1 hour per week once a week.  Most games will be played on Saturday morning.

Boys Sr. Basketball:  Boys Grades 6, 7 & 8

This program is for boys in grades 6, 7 & 8 .This is a fun yet competitive program, all abilities are invited to participate.  There will be a 1 hour practice per week and a 1 hour Saturday pre-season practice.  Games will begin in January and will be played on either Friday evening or Saturday.  All players who register by the deadline will be assigned to a team.All registration forms must be complete prior to participation in team selection. Financial aid is available, contact the Parks and Recreation Office for further information.

Season: Winter            Fee: $20 resident/ $30 non-resident                Registration Deadline:11/16/11

Pre-season clinic date: 11/19 10:30am-11:45am, 11/28 7:45pm-9pm & 12/3 11am-12:15pm

Day/Time:  Monday-Thursday 1 night/1 hour practice per week – with a Saturday per-season practice.  Games take place Saturday.

 Growin’ Grizzlies Floor Hockey:  Kindergarten Age Children

This is an exciting class to introduce the basic gym games to children in Kindergarten. This session will focus on floor hockey skills cooperative play as well as social interaction skills.  This class will take place on Wednesday with a morning and afternoon session, class is limited to 12 participants per session.  Floor Hockey will run for 5 weeks from November 10 - December 15.  Look for more Grown’ Grizzlies programs throughout the year.

  Growin' Grizzlies Skating Lessons: Kindergarten Age Chldren

This instructional program will teach the fundamentals of skating.  Children will need to supply their own skates, helmets are recommended.  Chldren will start by pushing chairs around the ice for balance and will eventually work towards skating freely by themselves.  Lessons will be outside and dependent upon the weather and ice conditions.

 Growin’ Grizzlies Basketball:  Kindergarten Age Children

This is an exciting class to introduce the basic basketball skills to children in Kindergarten. We will develop dribbling, passing, fundamentals of the game as well as social interaction skills.  This class will take place on Wednesday with a morning and afternoon session, class is limited to 10 participants per session.
